Ex vivo relative fitness was decreased for this double variant though the single variant (not found in autologous sequences), A146P had great fitness decrease, suggesting that I147L reverses this loss of fitness at position 2. The two mutations are linked.

Note Mutant ETINdEAAEW, was seen in patient PIC1362 580 DPS (days post symptoms) onwards but not at 769 DPS. It carried an escape at E207D and with significant fitness loss; when in tandem with an extra-epitopic mutation, V215L, 3 residues downstream of EW10. In patient 1483, it was seen at 2025 DPS as the double mutant E207D intra-EW10 and V215L extra-EW10 epitope. Of note - Patient 1362's CTL response was 10x that of Patient 1483! Patients PIC1052 and PIC1349 did not carry this mutation and PIC1052 carried the consensus L at position 215 extra-EW10 epitope. Data suggest that V215L augments cleavage of EW10, decreasing its presentation and increasing its escape function.

Note Single escape mutant NVTEeFNMW was seen in patient PIC1362 only at 344 DPS (days post symptoms)and had barely any loss of fitness. N92E replaced the N92D by 769 DPS dominating as a double mutant N92D/N94E which had restored viral fitness. Thus, the virus appears to evolve into a form that escaped with poor fitness to one that retained immune escape but improved viral fitness. Mutations at position 92 appeared to evolve sequentially. This might explain why the N92D/N94D escape variant of low fitness precedes the N92E/N94D fitness neutral escape.
