The impact of HLA class II alleles was studied on HIV disease progression. A black, Zulu/Xhosa South African, predominantly female, HIV-clade C-carrying, ART-naive cohort, the Sinikithemba, was studied and results were confirmed in a European-American male, HIV-clade B-carrying, untreated cohort. HLA DRB1*1303 was significantly associated with lower plasma viral load and an increased mean CD4+ T cell count, independent of ethnicity, sex and viral clade.
DRB1*1303, however, is associated with a decrease in IFN-γ-positive HIV-specific CD4+ T cells. Peptide pools spanning Gag, and Pol, Env, Nef, Acc, Reg were used to stimulate helper T cells and CTL; results are reported in Ramduth2009 and Kiepiela2004. The authors suggest that the protective activity of DRB1*1303 is mediated via an alternate mechanism that might limit the number of CTL available to HIV for infection.
