• In order to dissect whether natural control of HIV-1 infection is due to infection with a defective virus or whether it is due to host factors, 2 transmission pairs were studied and compared to an elite suppressor transmission pair with defective virus. The transmission pairs under study were Pair 1 = a male chronic progressor (CP1) and a female viremic controller (VC1); Pair 2 = a male chronic progressor (CP2) and female elite suppressor (ES38). Viral replication from CP1/VC1 and CP2/ES38 pairs was efficient, all patients had two wt CCR5 alleles, no homozygous protective HLA-C single nucleotide polymorphism (SNP) was found, none were positive for the KIR3DS1-activating allele, but ES38 is protective allele HLA-B*5703 positive.
• For the CP1/VC1 pair, it was found that VC1 had a higher percentage of CD4+ T cells that coexpressed γ-IFN and IL-2 secretions as well as CTL that coexpressed more perforin and TNF-α than CP1. CTL from VC1 substantially reduced infected CD4+ T cells in a suppression assay while CTL from CP1 had no effect.
• Epitope DQ15, DRLHPVHAGPIAPGQ, was targeted by both CP1 and VC1 but mutations were only seen in CP1.
