Notes

• To assess the contribution of CTL response to control of HIV-1 viral load and therefore transmission from mother to infant in breast milk, 2 HIV epitopes were studied in 10 HLA-A*3002 HIV-infected, breast-feeding Malawians. Right and left breast CTL response magnitude was similar though kinetics were discordant; but blood and milk had independently regulated response levels and kinetics regardless of escape. No correlation was found between CTL response and milk viral load or breast inflammation for both epitopes.
• Gag immunodominant epitope RY11 (RSLYNTVATLY) showed escape in > 50% subjects, with escape inversely related to CTL response and plasma or milk viral load. Median magnitude of response to RY11 was greater in milk than in blood but was similar in colostrum and mature milk.
