Notes

Mutational positions associated with escape and reversion were studied in a single subject (CH77) to determine their effects on viral fitness.
In the Env epitope QFRNKTIVF, the reversion mutation QFkNKTIVF was predominant in the patient's sequences at all timepoints from 14 to 592 days post-seroconversion. The mutant had the same fitness compared to the wild type sequence.
CH77 is known to be infected with Clade B HIV-1, consensus epitope at this location for ConB is QFgNKTIVF.

Notes

Diversification of env coding regions was examined in 81 very early B subtype infections. A subset of codon sites were identified that diversified more rapidly. In several cases, a rapidly evolving site was embedded in both an APOBEC motif and a CTL epitope, suggesting that APOBEC may facilitate early immune escape. Insertions and deletion mutations in env were also examined, with the finding that they arose by selection.
One patient had positive responses to previously-reported Cw*0401 epitope QFRNKTIVF and to the corresponding 18-mer REQFRNKTIVFNHSSGGD. The patient had negative responses to two common transmitted variant 18-mers, REQFkNKTIVFNHSSGGD and LSHVVDKLREQFkNKTIV.

Multifunctionality of CTL against transmitted or founder HIV-1 isolates during early or acute infection was examined for correlation with escape mutation selection. The earliest CTL responses were oligofunctional, and progressively became more multifunctional. A significant association was found between total CTL response and order of escape selection, and between epitope entropy with the rate of emergence of escape mutants.
18-mer HVVDKLREQFRNKTIVFNH (Gag 350-368) contains epitope QFRNKTIVF, that shows early evidence of escape by 14 days from onset of symptoms (DFOSx). This epitope elicited CTL response in patient CH77.

Using overlapping 15-mer peptides, CTL responses to early, acute HIV infection were determined. Early virus escape attributed to CTL response was charted and subsequent clusters of mutations due to NAb targeting were also found.
Five findings made: (i) exact numbers of transmitted HIV-1 actual and potential T cell epitopes were determined (ii) CTL-mediated virus escape occurring as viremia peaks, drops and reaches set point earlier and faster than previously known (iii) CTL responses to some transmitted virus epitopes were lost after early escapes were selected (iv) a few unexplained escapes were probably due to bystander mutations (v) earliest CTL responses cause the decline of VL in acute infection.
This transmitted, newly described epitope, QFRNKTIVF, was studied in patient CH77. A rapid change R355K was seen before day 14, with complete loss of the transmitted form.
Escape mutations seen were QFkNKTIVF, QFkNKaIVF and QFkdKTIVF. Other variants seen were QFnNKTIVF, QFsNKTIVF and QFRNKaIVF.
