To dissect whether the protective effect of HLA-B*57 in HIV-1 infection is due to CTL that recognize HLA-B*57-restricted epitopes or by the presence of viral-fitness attenuating mutations in such B*57 epitopes, 13 mother-infant pairs were studied. The B57 advantage is primarily through the maintenance of a fitness-attenuating mutation in p24-Gag, T242N in epitope TW10 (TSTLQEQIGW).
Regardless of parental inheritance, HLA-B57 controls VL even in infancy and even when escape mutations from B*57 positive mothers are transmitted. Mutations in p24-Gag are predominant in mismatched-HLA subject pairs and arise after vertical transmission, impacting viral fitness.
Epitope IAW9-RT, IATESIVIW, was recognized by 3 of 4 infants with paternally- and 1 of 4 maternally-inherited B57.

Viremic control among HLA-B*57 controllers was identified as being due to the lack of fitness-restoring viral capsid compensatory mutations (in response to escapes) and not superior T-cell responses.
11 >10-year child and adolescent survivors of perinatal HIV infection including 4 controllers showed that breadth and magnitude of CTL response is higher among progressors. Exclusive targeting of epitopes from Gag, RT was seen in controllers, but progressors also mounted response to Nef and other proteins' epitopes. There was no individual, specially targeted epitope in controllers.
Cytokine production by CTL is not superior in long-term surviving controllers, but correlates with the specific epitope that stimulates its release.
Wild type epitope IAW9 was seen in 4/6 progressors, subjects CH-01, CH-08, CH-11 and CH-10; and in 1/4 controllers, CH-04. CTL response to this epitope was seen in 5/7 progressors, CH-01, CH-08, CH-11, CH-10 and CH-12 as well as the one controller carrying the epitope, CH-04.

Peptide prediction programs based on MHC binding, proteasomal cleavage and TAP transport were applied to select novel epitopes absent from the Los Alamos HIV database. 22 novel epitopes were tested by IFN-gamma ELISpot and 19 (11 HLA-B27 restricted, 8 HLA-B57 restricted) induced strong positive responses.
Magnitude of response generated was similar to that of previously identified epitopes, but the number of individuals responding was lower. This could be explained by less stringent HLA-typing of subjects and perhaps high sequence variability within these novel epitopes.
IW9(Pol), IATESIVIW, is a novel HLA-B57-restricted epitope that elicits a CTL IFN-gamma response in the same range as Los Alamos database peptides.

Response to KF11 in B*5703-positive subtype C-infected subjects is associated with a lower viral setpoint, while response in B*5701-positive B-infected subjects is associated with a 10-fold higher viral setpoint. This difference seems to depend strongly on the residue at Gag173.
IATESIVIW was used as a control to show that B*5703-restricted IAW9-specific T cells had no differences in the level of stimulation by wild type NL43 versus Gag-S173T mutant NL43.

49 ART-naive chronically HIV-1 infected Chinese subjects' CTLs were tested with 413 OLPs in IFN-γ assays, determining optimal epitopes specific for Chinese Clade B HIV-1 sequences.
An inverse correlation was found between CTL response and viral load.
Targeted peptides have lower entropy than peptides that are not CTL targeted. This correlation is independent of HLA-restriction.
IATESIVIW, elicited an immune response in Chinese HIV-1 positive subjects as a part of peptide QKIATESIVIWGKTPKFK. This epitope differs from the previously described HLA-B58-restricted epitope IAMESIVIW, at 1 residue, IAtESIVIW.
8 of the 14 HLA-B58 carriers responded to IAtESIVIW-containing peptide with average magnitude of CTL response of 455 SFC/million PBMC (author communication and Fig.1).

From HIV-1 sequences of multiple subtypes, bioinformatic prediction generated 174 CTL epitopes from Gag, Pol, Nef, Env and Tat restricted by HLA supertypes A1, A2, A3, A24, B7, B44 and B58; 10 previously-published A2- and B58-restricted epitopes were also included. Of these 184 epitopes, 113 were recognized experimentally in serum from 31 HIV+ patients.
While Gag, Pol, Env and Nef were highly immunogenic, only 38% of Tat epitopes elicited a response.
IATESIVIW was a predicted epitope restricted by B58 supertype. 3/31 patient(s) responded to it; none of the responders were known to carry an allele of B58 supertype. Responding patients were infected with HIV clade(s) B, D.
