• This study investigated the efficacy of a therapeutic vaccine designed to boost CTLs and/or induce new CTL responses in HIV-infected individuals. The vaccine differed from other therapeutic vaccines that were previously studied in that it included not only clade B Gag, Pol, and Nef but also Env proteins from multiple clades (A, B, and C) and was followed by a replication-deficient adenovirus type 5 boost. 17 HIV-infected subjects were randomly assigned in a 2:1 ratio to receive vaccine or placebo. While vaccinated individuals exhibited as significantly stronger CTL response and broadened epitope-specific CTL repertoire, no changes were observed in frequency of latent infection or single-copy viral load.
• The epitope that stimulated the largest increase in CTL response was Clade A YI9, YPPIQGVI. All other epitopes elicited an unchanged or only slightly activated response.
• DI9, KDSQYALGII, was one of 9/48 vaccine-matched epitopes that were functionally tested. 6/9 of these, including DI9, were used in functional assays. These epitopes were identified in vaccine recipients by apheresis followed by CTL epitope mapping. Gag-specific responses were found to be most increased by vaccination though DI9 is an RT epitope.
