• HLA-B27 is associated with inflammatory spondyloarthropathies, but is also known to have the beneficial effect of a diminished course of certain viral infections like Influenza, Herpes Simplex type 2 (HSV2), Epstein-Barr (EBV) and HIV. This review covers host-factor effect on the presence of HLA-B27 in HIV patients.
• B27+ patients have CTLs selected for KK10, an immunodominant epitope from the conserved HIV protein, Gag p24. B27-KK10 is formed despite most mutations as the B-pocket of B27 creates a stable complex as long as an R is maintained at position 2 (P2). There is, unfortunately, an array of mutations that can escape and these are sexually transmittable and passed from mother to child.
