H4K3: Plasma from donor PG13 was found to have MPER neutralization activity, and mAb PGZL1 was isolated. When compared to a 4E10, PGZL1 was found to share similar crystal structure, contacts, and some common germline genes, but its neutralization and polyreactivity were less strong. Its structure also closely resembled that of VRC42.01. To identify more potent variants, heavy and light chain clones from the patient's repertoire were analyzed, and the sublineage variant H4K3 was shown to have stronger neutralization. A germline revertant of PGZL1 (PGZL1_gVmDmJ) was also isolated, and retained binding to MPER and weak neutralization despite DJ reversion.
